Coaching clinic and all its staff, paid or unpaid, volunteers and all other persons involved of

all responsibility whatsoever in any of the clinic activities for any accident or illness sustained

by participants during the course of the clinic.

     I further authorise the personnel mentioned above, in the event of any accident or illness, to

obtain such medical assistance as may be required and agree to meet any expenses incurred in 

providing the aforesaid medical attention.
